Team Registration

  

Teams are an integral part of the success of the Games! You already have distinguished yourself by expressing interest in forming a team, and we thank you for your athletic spirit!

Each company should designate at least one person to serve as Team Captain. Please notify Audrey Kerr at (202) 465-4118 or akerr@jdrf.org with the name of your company's 2008 Team Captain.

The Team Captain is the main point of contact for JDRF and is responsible for:

•
 Communicating instructions to the company’s participants
•
 Submitting event entries online and participant waivers via email or fax to JDRF by the registration deadline of May 28th.
•
 Collecting and accounting for all entry fees and submitting payment to JDRF by May 28th.


2008 Team Registration: ONLINE Registration will open on April 15th.
1. Download the Event Entry Form to determine your company's teams and participants for the various sporting events. Please consult the Rules & Event Descriptions for entry rules and a description of each sporting event.
2. Once you have finalized your company's participants, enter participant names online starting on April 15th. (Link to registration site given only to Team Captains – contact Audrey Kerr for the online registration link.)
3. Submit a signed Participant Waiver Form for each participant and alternate by either emailing them to akerr@jdrf.org, faxing to (202) 371-0046 or mailing the signed forms to the JDRF Capitol office.
4. Use the Team Payment Form to determine the total amount due and submit payment for participants and alternates ($30 per person per event) via credit card or check payable to "Juvenile Diabetes Research Foundation"

No one will be allowed to participate in this year's Games unless payment and the required forms are received by the deadline. There will be NO EXCEPTIONS and NO LATE PAYMENTS after May 28th. Sorry, NO REFUNDS for no shows the day of the Games.
